Please schedule your child's initial ADHD evaluation through our main office number (772)978-9000.  Before you schedule your appointment, we require that both parents (when possible) and as many teachers as possible fill out Vanderbilt Forms. You can download these forms at the links below and bring the completed forms to our office prior to your appointment. We will use these during the initial evaluation and periodically to monitor your child's progress. We would also like the school to complete a psychoeducational profile on your child whenever possible.

If your child is placed on medication, he or she will be followed closely in the office until a maintenance dosage is obtained.  At that point, we will require an office appointment approximately every 3 months. Your child's progress, as well as side effects to medication, will be carefully monitored using, in part, follow-up vanderbilt forms from parents and teachers.  These forms can be downloaded from the links below.
