We do sports physicals year- round.  They are usually valid for one year from the date of the physical but certain sports have particular requirements. Please check with your coach regarding your specific requirements before you schedule your appointment. You need to complete Page 1 of the Sports Physical form prior to your appointment and bring both pages to your physical.  If you do not have a sports physical form, you may download it from the link below. If you have already had a physical and need to request a sports form be completed by our office, please complete side 1 of the form and drop it off at our office to be completed.
